| AGREEMENT: I agree to the terms of use for the Pinnacle Institute membership. I agree to allow Pinnacle to collect monthly payments in advance for this Pinnacle membership with the payment information provided on this application. I understand that the charges will appear on my statement as "PINNACLE ONLINE TRAINING". I agree to notify Pinnacle in writing in the event that I wish to discontinue my Pinnacle membership. I understand that any charges on my billing statement will be from Pinnacle. I agree that if I have a Business Membership, I will ensure that only one individual and their immediate family members utilize each individual member account. |

| RETURN POLICY: I understand that if I am not completely satisfied with the quality and value of my performance membership that I may cancel my membership within the first 30 days to receive a 100% refund of my monthly membership tuition. I also understand that I may cancel my membership in writing at anytime up to five (5) days before my monthly billing cycle and I will not be subject to any further billings. |

| I hereby authorize Pinnacle to initiate credit and debit entries to the specified account(s) at the specified depository(s). This authorization is to remain in force until Pinnacle has received written notification from the member of its termination in such time and in such manner as to afford Pinnacle and the depository reasonable opportunity to act upon it. |
